MODEL RELEASE FORM (DIANA MILLER PHOTOGRAPHY)

In consideration of the terms hereof, I hereby grant DIANA MILLER PHOTOGRAPHY, the Photographer, and those acting with her authority and permission, the absolute right and permission to use my name and the photographs for the purpose of display, portfolio, advertising, website, prints and any publication to promote her photography.

I agree that I have no right to sell, license or publish any of the photographs to any person or entity. I waive any and all claims, demands, actions and causes of action whatsoever arising out of or related to any loss, damage, or injury that may be sustained by me or any of the property belonging to me.

I state further that I have read the above authorization, release and agreement prior to its execution, and that I am fully familiar with the contents thereof. This release shall be binding upon the minor and me, and our respective heirs, legal representatives, successors and assigns.
